Mike stated that he had just completed a site tour / review of confined space activity when he received a call on the radio. Being unfamiliar with the facility locations (terminology), he requested directions from the Control Room Operator. He took his truck to the location of the incident and began assessing the situation. Upon determining that there was a person left inside the oxygen deficient space, he donned his SCBA and entered the space to retrieve the victim. Upon entering the space, he saw the victim trying to stand. He offered direction and assisted the victim outside of the space into fresh air.

Shannon (Safety Supervisor) noticed an increase in Benzene levels and felt it was was not safe. He contacted me and told him the levels were too high for the guys to continue to work safely. He then offered a alternate plan to get the job done. Shannon then called back to Code Red Safety and had set up fit testing, ordered an air trailer. Within 48 hours crews were back working on the project again safety. We loved the fact that the safety supervisor just did not come up to us and shut down the job. The safety supervisor actually had a plan to keep the project running. Matt Kehoe
